How Clexed checks its content
In short: Clexed's questions and lessons are written with AI and checked by automated tests and by a second, independent AI system. They are not reviewed by a nurse educator. Use Clexed alongside your program's materials, and report anything that looks wrong.
What we cover today
- 790 practice questions and 14 unfolding case studies, mapped against the 2026 NCLEX-RN test plan's 8 client-need areas.
- 328 of the test plan's 511 content statements (64%) are tested by at least one question so far. We publish this number because we are still filling the gaps.
- 116 lessons and guided revision sessions.
How a question gets in
- Automated checks on structure, scoring, wording and every dosage calculation (answers are re-computed by code).
- A second AI system from a different company answers each question with the key hidden and the options shuffled. If it disagrees, or doubts the answer, the question is fixed and checked again, or removed. Hidden test questions with proven answers check the checker.
- The clinical facts our questions and lessons rely on are kept in a knowledge base of 1,794 records. Each one quotes, word for word, a published source we are allowed to use: US government sources (such as the CDC, NIH and FDA drug labels) and openly licensed nursing textbooks. Our questions and lessons are checked against it automatically.
- 18 high-stakes facts (for example potassium limits for insulin in DKA, transfusion rules and CPR rates) are also tracked so that outdated values are found everywhere they appear.
- Lessons and guided sessions are fact-checked the same way, with web search, and taken down until any reported error is fixed.
What this does not prove
- Two AI systems can share the same mistake. That is why key facts are checked against published sources and why your reports matter.
- It is not clinical validation.
Who we are
Clexed is an independent study website built to help you prepare for and pass the NCLEX-RN. It is not affiliated with or endorsed by NCSBN. NCLEX® and NCLEX-RN® are registered trademarks of NCSBN. Our learning objectives are written by us and mapped to the published structure of the 2026 test plan.
- The readiness score is a practice estimate. It has not yet been checked against real exam results, so it does not predict whether you will pass.
If something looks wrong
Tap Report on any question or lesson. If you are signed in and report a wrong or outdated answer, or two people report the same question for any reason, it comes out of the bank straight away while we check it. Other reports are reviewed within 24 hours.
